Breast Augmentation is an operation to increase the size and shape of a woman’s breast. I have been performing this surgery in New York City for over 20 years. Many of our patients note an increased sense of self confidence and improvement of their appearance after Breast Augmentation. I however, always stress that Breast Augmentation should be done for you only, and not for your husband or boyfriend.
It is your body and the decision for Breast Augmentation should be yours as well. Expectations for breast surgery should be healthy and realistic. In my practice, patients come in all shapes and sizes, with desires ranging from “just a bit bigger” to “VaVaVaVoom!” I try to temper patient wishes and desires with reality, like patient anatomy, body type and skin elasticity. It is my job to counsel, educate and listen to my patients, to help me achieve the best possible result, and of course, make sure my patients are happy with that result!
Breast Augmentation Consultation
The consultation process (plan to spend at least one hour), includes an in-depth discussion about your desires with regard to size, shape and dimensions of your breasts after Breast Augmentation Surgery. It is the surgeon’s task to explain all aspects of the operation, the pros and cons, and most importantly, the concept of realistic expectations with regard to your postoperative result.
I ask each patient to bring photographs demonstrating the size and shape of the breasts that she desires, so I can understand exactly what she is looking for. I use various techniques to achieve this goal, including the process of choosing assorted sized breast implants and placing them in brassieres covered by T-shirts, to give you an idea of what you will look like in clothing after surgery with different sized breast implants.
I prefer to meet a second time with each patient prior to surgery to assure complete understanding of desires from her breast augmentation operation. At this second consultation we review the photographs of breasts and size that you desire. This helps me to clarify your desires, as dimensions such as brassiere sizes varies from manufacturer to manufacturer, and can be most confusing.
Ideal Patients for Breast Augmentation Surgery
Ideal patients are those in good health, have realistic expectations and have researched and understand all aspects of the operation and recovery. Those women seeking to increase their breast size realistically, replace loss of volume from pregnancy or weight reduction or seek to balance their body contour are excellent candidates and happy postop patients.
Breast Augmentation Surgery and Options
There are 3 possible types of incisions for this operation; directly below the nipple, within the crease below the breast, or in the armpit region. The implants may be saline or silicone, round or shaped, smooth or textured. They can be placed above or below the chest muscle under the breasts. I am comfortable with all of these incisions and approaches, and I use all types and shapes of implants.
I select implants to tailor the operation to the patient’s desires, anatomy and body type. Nearly all breast tissue may be visualized in mammograms with the implants under the muscle; slightly less so when over the muscle, but breast imaging techniques and technology have improved to such a degree, that most radiologists are extremely comfortable imaging patients with breast implants.
The operation is performed in my state of the art operative suite on the Upper East Side in New York City under general anesthesia or sedation with local anesthesia, administered by a board certified anesthesiologist. The surgery takes approximately 2 hours, followed by a short recovery with a nurse at your side.
Breast Augmentation Surgery Recovery
For your safety, I insist that a friend, family member or private duty nurse accompany you home and stay with you for at least one or 2 nights. Postoperative pain is effectively managed with pain medication, and most of my patients return to work in less than one week. No heavy lifting or vigorous physical activity for 4-6 weeks.

Breast Augmentation Surgery FAQs
What is the advantage of silicone versus saline implants?
The modern generation silicone implants are soft, safe and have an extremely similar feel to breast tissue. They are most effective in women with little or no breast tissue who are very thin, and in those that require the implants to be placed under the breast tissue, over the muscle. Saline implants are still most effective in many women, especially those with modest amount of breast tissue, and those women desiring implants under the muscle.
How long do breast implants last?
Manufacturer’s recommend replacement of silicone implants every 12-15 years. Saline implants may be kept in place until the leak or deflate, because they are filled with saltwater, which your body absorbs. However, they usually last approximately 12-15 years.
How long can I expect to be swollen after undergoing Breast Augmentation Surgery?
The majority of swelling will decrease over the first 4-6 weeks, but complete resolution takes approximately 3 months.
When we will my recovery be complete?
It usually takes approximately 6 months for the muscle tissue, breast tissue and skin to stretch and accommodate the implants and allow them to settle in. The breasts look good right after surgery, but improve over time, and by the end of 6 months most of my patients tell me that they truly feel like the implants are theirs.
What kind of scars can I expect after having a Breast Augmentation Procedure?
Each of the approaches described above result with scars that are placed in locations that are hidden and well camouflaged. However, each person heals differently and scarring varies from individual to individual. My staff and I counsel all our patients regarding scar care after surgery.
